Coordinated Outreach Ends Long-Standing Encampment

A coordinated, multi-agency effort led by the Key to Home Partnership successfully closed a homeless encampment where 45 people were living and moved every person into a stable, permanent home. The operation brought together nonprofit caseworkers, city housing staff, outreach teams and landlords to speed placements.


Case Management and Wraparound Services for Stability

Each person placed receives an assigned case manager who helps them:

  • Secure and maintain ID documents (birth certificates, state IDs, Social Security cards)

  • Enroll in benefit programs (SNAP, Medicaid, rental assistance)

  • Find job training and employment opportunities

  • Establish lease agreements and receive landlord mediation support

Outcomes, Follow-up, and Next Steps

Program leaders say follow-up checks and employment supports are scheduled at 30, 60 and 90 days to reduce returns to homelessness. The Key to Home Partnership is using this success to pitch similar coordinated efforts in other regions.
